All Together Fairer: Our Health and Care Partnership Plan 2024-2029

Cheshire and Merseyside Health and Care Partnership (HCP) has published its plan for 2024-2029.
The plan sets out how we will work together to address the key challenges facing people across Cheshire and Merseyside.
All Together Fairer: Our Health and Care Partnership Plan is built around four core strategic objectives:
- Tackling health inequalities in outcomes, experiences and access (our 8 Marmot principles)
- Improving outcomes in population health and healthcare
- Enhancing productivity and value for money
- Helping to support broader social and economic development
The creation of our HCP provided the platform from which all partners can challenge their mindsets, share learning and work differently to optimise our collective contribution to people’s lives.
Tackling health inequalities is our shared key aim. As a ‘Marmot Community’, we are truly committed to improving the health and wellbeing of our population and, in doing so, focusing on reducing inequalities.
Cheshire and Merseyside HCP is committed to involving people and communities to identify what will help to improve their health and wellbeing and to work with us to shape services.
We will work to develop this plan and deliver the detailed work which sits behind it, ensuring the voice of our communities is at the heart of everything we do.
